Dear Sir or Madam,
This is to confirm my verbal conversation with the IRS office that you have put all IRS collection efforts on hold so that the IRS has ample time to consider request the below penalty abatement request. I am writing you in a good faith attempt to come to terms over the above taxpayers’ present predicament with the IRS, get their returns filed, and to address their tax issues.
As you can see by the attached Statement that the taxpayer has paid their taxes in full along with their estimated interest, while enclosing an affidavit to request abatement for the penalties. Accordingly please cease all collection efforts on this account while our request is being carefully considered and evaluated.
The $15,000 enclosed check is to be applied as follows and in accordance with the details on the attached Statement. Please note the attached Statement is detailed by year and amount whereas the below recap is only by tax form and type. Accordingly please be sure to apply the $15,000 check as per the attached Statement.

Please accept these monies and as full payment against the principal portion and estimated interest of their account balance while abating the assessed penalties. Upon learning of the shortfall, the taxpayers immediately paid the tax and interest in full. The taxpayers have provided just cause (copy of affidavit enclosed) for the initial failure and have now made/paid the taxes and interest. I appreciate your attention to this matter and look forward to working with you to close their file.
